Team Clothing Reimbursement Form

Athletes on the Ontario Water Ski Team can use this form to request a clothing reimbursement after purchasing official team gear. Provincial Team members are eligible for up to $100, and Development Team (Tier 2) members are eligible for up to $66. Unfortunately, Tier 3 (Grassroots Team) athletes are not eligible for this funding.

Please note: Only items purchased for the team athlete are eligible for reimbursement. Any clothing purchased for friends or family members will not be counted toward the total.
